Change Of Beneficiary

Many policyowners want to change the name of the beneficiary or include more beneficiaries on their life insurance policy. The process is simple. You may contact your financial professional for assistance. You may also call our toll-free Customer Service telephone number at 1-800-283-2959 and request a Beneficiary Change Request form. We will mail it to you along with instructions on how to fill it out. (A copy of this form may also be printed from the Online Forms section of this site.) Please reference the Legal Matters brochure to help you determine appropriate beneficiary changes.

Once you have the Beneficiary Change Request form, you should complete it as soon as possible and return the form to us by mail. You should retain your copy of the Beneficiary Change form with your life insurance policy.
